To initialise it to 1 before each of the two &quot;for each&quot; loops:

	&lt;strong&gt;$question_count = 1;&lt;/strong&gt;
	foreach ($all_question as $ques) {

To increment it at the end before exiting the loop:

		&lt;strong&gt;$question_count++;&lt;/strong&gt;
	}
